It’s the most reliable way we have to detect whether gas-producing bacteria have overgrown in the small intestine - and whether the overgrowth is primarily hydrogen, methane, or hydrogen sulfide.

Each of these types requires a slightly different treatment approach. Without this clarity, it’s easy to go in circles.

Why testing matters

Bloating, reflux, constipation, diarrhoea, food sensitivities - these symptoms are common in SIBO, but they’re not exclusive to it. Parasites, Candida, H. pylori, LIBO, and even nervous system issues can cause a similar picture. That’s why testing is so important.

A proper SIBO breath test helps us:

  • Confirm whether SIBO is actually present

  • Identify the type of gas (hydrogen, methane, or hydrogen sulfide)

  • Pinpoint whether the overgrowth is in the small or large intestine

That information allows us to tailor your treatment, rather than relying on guesswork - which can save you months (or years) of frustration, unnecessary diets, and trial-and-error protocols.

The test we recommend

We generally recommend a 3-hour lactulose breath test. Lactulose is a non-absorbable sugar that travels the entire length of the small intestine, feeding any bacteria along the way and triggering gas production (hydrogen and/or methane) if overgrowth is present.

This gives us the clearest picture of what’s happening not just in the upper part of the gut, but throughout - which is especially important when we’re trying to distinguish between SIBO and LIBO (Large Intestinal Bacterial Overgrowth).

Shorter tests or those using glucose can miss key information, especially if the overgrowth is further down the small intestine.

A 3-hour lactulose test reduces the chance of a false negative and helps us tailor treatment more precisely.
